Apply to IPCN: contractor checklist

Use this checklist before you start the online application — it matches what the office checks during review.

Before you apply

Confirm you are an independent pharmacy contractor (or eligible applicant per our criteria) and have your GPhC registration details to hand.

Gather company information if applicable, Companies House number, officer details and pharmacy branch names you operate.

Prepare to accept the current membership agreement version shown in the application; acceptance is recorded with timestamp.
